We provide Homemaker Personal Care; Home Health Services; Social Work; CPR/First Aid; Adult Day Habilitation.

We improve the quality of health care service delivery for all individuals and end significant health disparities of race, income and geography by providing a standard of excellence in residential and community settings while giving resources, advocacy, care and equality.

Our Purpose: To ensure individuals have every opportunity to become resilient and achieve an optimal level of living while contributing to their overall health and well-being.

We are happy to serve Children (18 & under), Adults and Seniors (65 & older) through Self Waiver, I/O Waiver, & Level 1 Waiver for Behavioral/Medical; and soon to come through the Ohio Home Care Waiver program.
